The mission of OC Anaheim ADHC is to provide high quality physical, emotional, and cognitive health care and supportive services to enhance the wellbeing of frail elderly by providing an engaging and stimulating setting that enhances uniqueness, dignity and joy of each person.

OC Anaheim ADHC has a multidisciplinary team of health professionals that conduct a comprehensive assessment of each client to meet the individual's health and social needs. Assist family members and caregivers to take care of the elderly and take some time off.

Qualification
- Resides in Orange County
- Adults aged 18 or older with active Medi-Cal coverage
- Individuals with chronic illnesses, disabilities, or physical and/or mental health conditions requiring medical care
- Elderly individuals who live alone or whose children are unable to provide care due to limited time
- Seniors with a history of emergency department visits or hospitalizations
